Flat-topped dormers built, re-covered and led in. Roof, cheeks and the joint back into the main pitch.
Tiles shed water perfectly well on their own. It is the places where a roof meets something else — a wall, a chimney, another pitch, a dormer cheek — where water finds its way in.
Those joints get lead. Cut to size, dressed to the shape of what is underneath and fixed properly. Not a bead of mastic and a hope that it lasts until someone else is looking at it.

Access is most of the cost on this kind of work. If the guttering is tired and the fascias are going, doing them at the same time as the roof saves paying twice to get up there.
